If you have been unwell for 7 days or less, you don’t need to see a clinician. You can complete a Self-Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances may involve a charge of £10.

Many employers have their own self-certification forms. If your employer doesn’t have its own form, you can complete a Self Certification Form. Please fill it in and give to your employer. You do not need to visit your GP surgery for this.

If you have been unwell for more than 7 days, you will need a medical certificate. These certificates are called ‘Statement of Fitness for Work’ Certificates (Fit Note).

Please complete an online consultation to request a certificate from the surgery.

A clinician may contact you or arrange for an appointment to discuss your condition further, before a certificate is issued.
